Hardship applications MUST be complete and MUST include all things required and asked for within the application or it cannot be considered complete and may be denied.

NOTE:  Our billing company, Medical Reimbursement Services (MRS), will only send an invoice.  HOWEVER, our collection company, should you become delinquent, will send text messages with a link to make a payment.
